These Terms of Service ("Terms") govern your use of the Patient Coverage Connect platform and website at patientcoverageconnect.com, operated by Patient Coverage Connect LLC ("PCC," "we," "our," or "us").
By using our website, receiving communications from PCC, or having a referral submitted through a participating healthcare provider, you agree to these Terms. If you are a healthcare provider or insurance professional accessing PCC through a partnership agreement, separate terms may also apply.
Patient Coverage Connect LLC is a clinical referral platform. We help healthcare providers identify Medicare coverage questions for their patients and connect those patients with licensed Medicare insurance advisors through Field Marketing Organizations (FMOs). We are a referral and coordination platform — we are not an insurance company, insurance agent, or FMO.
PCC does not sell insurance. The Medicare advisor you are connected with through PCC is a separately licensed insurance professional. Any insurance plan you discuss, enroll in, or decline is entirely between you and that advisor. PCC has no role in plan selection, enrollment, or insurance policy terms.
When a clinical staff member at a participating healthcare provider submits a referral on your behalf — following your verbal permission — you are consenting to be contacted by a licensed Medicare advisor. By confirming an appointment through the scheduling link in your notification, you are providing express written consent under the Telephone Consumer Protection Act (TCPA) to be contacted by the specific named advisor shown at the time of your confirmation.

You may withdraw consent at any time by replying STOP to any text message or contacting us at privacy@patientcoverageconnect.com. Withdrawing consent does not affect the lawfulness of any contact that occurred before withdrawal.
Patient Coverage Connect LLC may send you text (SMS) messages in connection with a referral submitted on your behalf. By having a referral submitted through a participating provider, you consent to receive these messages.
We send transactional and informational text messages related to your referral, including appointment confirmation, advisor identification, and scheduling reminders. Message frequency is typically one to three messages per referral event.
Reply STOP to any message to opt out. You will receive one final confirmation and no further messages. Reply HELP for assistance.
Message and data rates may apply. PCC does not charge for messages, but your mobile carrier's standard rates may apply.
Messages are delivered through major U.S. carriers. Delivery is not guaranteed on all carrier networks.
Patient Coverage Connect LLC is not responsible for any charges incurred from your mobile carrier in connection with text messages sent through our platform.
Nothing on patientcoverageconnect.com or in any communication from PCC constitutes medical advice, insurance advice, or a recommendation to enroll in any specific Medicare plan. Coverage gap alerts and referral summaries are informational only.
Medicare coverage decisions are personal and depend on individual circumstances including your health needs, finances, and provider network preferences. PCC connects you with a licensed advisor; all coverage decisions are yours to make with that advisor's guidance.
If you are a healthcare provider accessing PCC through a provider partnership, your use is also governed by the Business Associate Agreement (BAA) and any applicable partnership agreement between your organization and PCC. In the event of a conflict between those agreements and these Terms, the BAA and partnership agreement control.
Providers using the PCC referral workflow are responsible for:
Licensed Medicare advisors and FMO partners accessing PCC leads or the PCC platform are subject to the separate FMO partner agreement. In addition, all advisors accessing PCC leads agree that:
The Patient Coverage Connect LLC platform, website, workflows, referral routing methodology, and all related technology are proprietary to PCC. Nothing in these Terms grants any license to PCC's intellectual property except the limited right to use the platform for its intended purpose.
You may not reverse engineer, copy, reproduce, or attempt to replicate any aspect of the PCC platform or its underlying methodology.
The PCC platform and website are provided "as is" without warranties of any kind, express or implied. We do not warrant that the platform will be uninterrupted, error-free, or that any referral will result in a Medicare enrollment or a satisfactory outcome.
To the fullest extent permitted by law, PCC's liability for any claim arising out of use of the platform is limited to the fees, if any, paid by the claimant to PCC in the twelve months preceding the claim. PCC is not liable for any indirect, incidental, consequential, or punitive damages.
PCC is not responsible for the actions, advice, or conduct of any Medicare advisor connected through the platform. The advisor-patient relationship is separate from PCC's role as a referral platform.
These Terms are governed by the laws of the State of Tennessee, without regard to conflict of law principles. Any dispute arising from these Terms or your use of PCC shall be resolved in the state or federal courts located in Hamilton County, Tennessee, and you consent to the jurisdiction of those courts.
We may update these Terms from time to time. We will post any changes on this page with a revised effective date. Your continued use of the platform or website after changes are posted constitutes acceptance of the updated Terms.
