1. About Patient Coverage Connect LLC

Patient Coverage Connect LLC ("PCC," "we," "our," or "us") is a clinical Medicare referral platform that connects patients with licensed Medicare insurance advisors through partnerships with healthcare providers. We operate inside clinical settings — physician offices, pharmacies, and community health centers — where our platform helps identify Medicare coverage questions and routes patients to qualified advisors for follow-up.

This Privacy Policy describes how we collect, use, share, and protect information in connection with our platform and our website at patientcoverageconnect.com.

2. How PCC Handles Clinical and Health Information

Important: Patient Coverage Connect LLC does not store Protected Health Information (PHI) on its own servers. When a referral is submitted, clinical data from your healthcare provider's record is parsed in our system's application memory to build a referral packet, then routed — but never written to or retained in PCC's own storage. Your full medical record remains with your provider.

PCC operates as a Business Associate of participating healthcare providers under the Health Insurance Portability and Accountability Act (HIPAA). Our access to and use of health information is governed by Business Associate Agreements with each provider partner. We do not sell, lease, or otherwise commercially exploit PHI.

The referral packet we transmit to your assigned Medicare advisor includes limited information relevant to your coverage question — such as your name, phone number, date of birth, relevant medications or plan details, and the nature of your coverage question. This information is transmitted to a licensed Medicare advisor through their Field Marketing Organization (FMO) platform so they can assist you.

Website and Platform Usage

When you visit patientcoverageconnect.com, we may collect standard web analytics data such as pages visited, time on page, referring URL, browser type, and general location. We do not use this data to identify individual visitors.

4. How We Use Your Information

We do not sell your contact information. We do not share your information with lead aggregators, data brokers, or advertising platforms. Your referral is routed to one licensed Medicare advisor for the purpose of the coverage question you raised with your provider.

5. Text Messages (SMS)

When a referral is submitted on your behalf, you may receive a text message from Patient Coverage Connect LLC notifying you that a Medicare advisor will be following up. This message will identify the advisor and provide an opportunity to confirm your appointment or opt out.

You may opt out of text messages at any time by replying STOP to any message. After opting out, you will receive one final confirmation message and no further texts. For help, reply HELP.

Message frequency is one confirmation message per referral, plus any STOP or HELP replies you request. Standard message and data rates may apply depending on your mobile carrier plan.

We do not share your mobile number with third parties for their own marketing purposes. Your number is used solely to facilitate the referral you consented to through your healthcare provider.

6. Sharing Your Information

We share your information only in the following circumstances:

With Your Assigned Medicare Advisor

A referral packet containing your name, contact information, and coverage question details is transmitted to the licensed Medicare advisor assigned to assist you, through their FMO platform. This is the core function of the PCC service and the purpose for which your provider submitted the referral.

With Service Providers

We use third-party services that process data on our behalf, including SMS delivery, consent documentation (TrustedForm by ActiveProspect), and DNC compliance screening (Blacklist Alliance). These vendors are contractually bound to use your information only to perform services for PCC.

As Required by Law

We may disclose information if required by law, subpoena, or other legal process, or if we believe in good faith that disclosure is necessary to protect our rights, your safety, or the safety of others.

Business Transfers

If PCC is acquired, merges with another company, or transfers its assets, your information may be transferred as part of that transaction. We will notify affected users by posting a notice on our website.

7. Data Retention

Consent records, referral documentation, and DNC compliance records are retained for a minimum of five years to satisfy regulatory requirements under TCPA, CMS Medicare marketing rules, and state insurance regulations.

We do not retain raw clinical data. The referral packet transmitted to the advisor's platform is subject to that platform's own data retention policies.

8. Your Rights

Depending on your state of residence, you may have rights regarding your personal information, including the right to access, correct, or request deletion of information we hold about you. To exercise any of these rights, contact us using the information below.

If your information was shared with a Medicare advisor's platform, you may also need to contact that platform directly to exercise rights regarding their records.

9. Security

We implement administrative, technical, and physical safeguards appropriate to the sensitivity of the information we handle, including encrypted data transmission, access controls, and infrastructure designed to meet HIPAA security requirements. No system is completely secure, and we cannot guarantee absolute security.

10. Children's Privacy

Patient Coverage Connect LLC is a Medicare referral platform designed for adults aged 64 and older approaching Medicare eligibility, or adults already enrolled in Medicare. We do not knowingly collect information from individuals under the age of 18.

11. Changes to This Policy

We may update this Privacy Policy from time to time. We will post the updated policy on this page with a revised effective date. If changes are material, we will provide additional notice. Continued use of our platform after changes are posted constitutes acceptance of the updated policy.
